Based on your preferences, we provide you with information about one or more of our contracted senior living providers ("Participating Communities") and provide your Senior Living Care Information to Participating Communities. The Participating Communities may contact you directly regarding their services. APFM does not endorse or recommend any provider. It is your sole responsibility to select the appropriate care for yourself or your loved one. We work with both you and the Participating Communities in your search. We do not permit our Advisors to have an ownership interest in Participating Communities.
II. How We Are Paid. We do not charge you any fee – we are paid by the Participating Communities. Some Participating Communities pay us a percentage of the first month's standard rate for the rent and care services you select. We invoice these fees after the senior moves in.
III. When We Tour. APFM tours certain Participating Communities in Washington (typically more in metropolitan areas than in rural areas.) During the 12 month period prior to December 31, 2017, we toured 86.2% of Participating Communities with capacity for 20 or more residents.
IV. No Obligation or Commitment. You have no obligation to use or to continue to use our services. Because you pay no fee to us, you will never need to ask for a refund.
V. Complaints. Please contact our Family Feedback Line at (866) 584-7340 or ConsumerFeedback@aplaceformom.com to report any complaint. Consumers have many avenues to address a dispute with any referral service company, including the right to file a complaint with the Attorney General's office at: Consumer Protection Division, 800 5th Avenue, Ste. 2000, Seattle, 98104 or 800-551-4636.

Are you talking about the letter you get when Medicaid has been approved? That letter is usually sent to the address of the person applying for Medicaid. If your talking about the yearly renewal, again, that is sent to the address of the person who applied for Medicaid. My nephew gets Healthcare thru Medicaid. He just renewed and the nenewal letter went to his home. For my Mom, once she was in LTC the letter went to the facilities office.
If you are on Medicaid, you have a caseworker and they should be able to send you a copy of your reward letter. Just call your County Social Services and tell them you need a copy.
Are you on Medicaid, if not to get an award letter you must apply for Medicaid and be approved. That could be for in home health, healthcare or Longterm care. Call your County Social Services to apply.
Are you referring to the “award letter” that your State Medicaid’s Long Term Care program wants submitted as a part of the required application documents? So you are filing for this program as you are needing to be in a NH or another type of care facility? Or you are a POA or family member doing the application for an elder family member? Is this it?? Is this the type of awards letter needed???
If this is “award letter” you are needing, it is something that is snail mailed between November - December each year. It technically is called the Benefit Verification Letter that states what will be paid for the incoming year. If you are getting Social Security retirement income, it is a tri-fold double sided document sent by SSA to whatever address they have on file for you. Other retirements (pensions, federal employee retirement annuity, etc.) also send out their own benefits to be paid for the incoming year letter btwn Oct - Dec. All letters basically state the precise $ amount “awarded” or paid for the next year.
LTC Medicaid absolutely needs these awards letters as they use it to verify and determine if your income does not exceed their State maximum for eligibility. fwiw most States income max is $2,982 for the LTC Medicaid program. Medicaid caseworker uses the info to determine what the exact $ amount will become your Share of Cost or copay due to the NH every month as per LTC Medicaid rules.
Also SSA awards letter will also show if any deductions automatically taken from the monthly SS payment. Like for many on SS retirement income, there is a deduction of $202.90 each mo to pay Medicare Part B premium. However, if you are going onto LTC Medicaid, most have that Part B premium stopped as they will be now going onto their States Medicaid for secondary health insurance. So that $202.90 becomes available to use to pay the required Share of Cost or copay to the NH each month.
(Those on LTC Medicaid end up on 3 programs: as “duals” for health insurance via Medicare and Medicaid AND on the LTC Medicaid program that covers their custodial care costs in the NH facility).
If you cannot find the letters, then you will have to request them. For SSA, it’s done thru your online account. If you are doing this for a family member, you will have to help them do their own online account or help them access it. Go to the “Replacement Document” section and it will have a link to get a copy of the awards letter. You download it and print it. Online 100% quickest way to get it.
If there are also pensions or other retirement income, that’s usually contacting the HR dept or retirement benefits dept or maybe an outside company who handles pensions. Most have an online system like what SSA does.
IMPORTANT! for more fun in this, when LTC Medicaid is renewed next year, IT WILL NEED THE AWARD LETTER sent out in Nov-Dec of this year. So be on the lookout for it and set it aside…. along with bank statements and any other income / resources paperwork that comes in during the year.
