I am concerned about my ability to pay for necessary back/nerve care & mental health care and medications in the future. These are the two main factors considered to be my disabilities. I don't feel Medicaid will adequately provide the level of care I need. Is it possible to put the back pay money in a medical trust to pay for future medical needs not covered by Medicaid?

What will happen, you're backpay will reflect how many months you were in the NH, especially if Medicaid paid for more than half of your state.

For example if you were in the home for 9 months and the most you can get from SS is $30 (maybe $50 in some states) part of your backpay would be $270 (30 x 9). The only way you could have no interruption is expressing need and that your stay was only for 90 days or less and you had a note from your physician stating that.

Any backpay you're entitled to will show this amount for being in the NH, when they calculate your final backpay amount.

In the last year or so, SS will only backpay 1 year no matter when u applied. Also, the lawyer handling your case will receive 25%, I think it is now, of that back pay.
